As it says above. As a player, in what circumstances are you willing to see your characters lose?
At any point they do something stupid and reckless. I include situations the players are not meant to go anywhere near in my games. If they can not figure that out or choose to run at something based on, "We're the players so we have to win" then their lives are likely to be quite short.

Players need to have fun but they don't need to "win." Thankfully RPGs aren't primarily concerned with "winning" vs. "losing."

Some of it I think depends on the feel or tone of the game. If it is a gritty, realisitic horror game, then yes they may or likely will fail. If it is an over the top, action game of highly competent people then they likely will often succeed with only the occassional failure. With many and varied shades in between them. Like so many things there really aren't any black & white answers to questions like this.
I don't care if my character loses, the chance of losing is part of the fun or risk of playing and makes winning all the sweeter. If the loss has fatal consequences for my character however I do like to bow out spectacularly, if it's possible.

Anytime "losing" is fun. Which usually means it is in character for one reason or another...there are some ideals worth laying down your character's life. What they are may well change from one character to another...
