That last point rings very true. Among my player group, there have been some pretty severe out of game run ins between players, and sometimes between players and myself. I GM most games I am involved with, and I do not allow any problems I have with players to affect the game and their characters. A very important point you have raised here.

On the face of it, yes, but as you probably know, these days RPG campaigns may span a decade or more of real time and sometimes players and GMs alike have a lot of emotional and time investment wrapped up in games. In my case, several of the players in my campaign had big personality clashes with one another long before I had any problems with any of them. It added a certain spice to their in-game interactions.
