This is certainly an issue you should consider, although I think a good gamemaster can easily control the level of danger the players see. Your description makes it sound like the bloody outcomes of TROS combat are random—i.e., that a player has little hope of surviving a half dozen fights because each is a flip of the coin. This might be true if the fights are well matched and people lack heavy armor. However, dorky mooks can be slain in large numbers in this game without forcing the players into dangerous situations. This issue will also be strongly influenced by player skill; you must know what to do with your advantages if you are to dominate weaker NPCs.

Finally, I agree that D+D or other abstract, cinematic games are by far the best choice for certain kinds of campaigns. TROS (as patch or stand alone game) is really only suited to game worlds in which players and NPCs live by similar, relatively realistic rules. This kind of campaign works well in many settings (including middle earth), but you have to go into it with eyes open.
