They do both, as do all baility uses. Think of it this way:
Every time a characetr uses an ability, you describe what they did in the game world, and what the effect was in the game world. e.g. "I kick Baldrick in the nuts - success! he doubles up in pain.".
Also every use of an ability in an extended contest potentialy affects the current Action Points the characetrs have "I kick Baldrick in the nuts - success! He loses 10 APs."
These are the same thing, they happen in paralell to each other. One describes the results in game world terms, the other in game mechanics terms. This is no different from any other RPG, in theory.
With feats, you perform the feat (e.g. leaping over some trees), and this has an effect on how many APs you or your opponent have. e.g. maybe you gain a lot of APs in your attempt to escape your enemies, if you're leaping over the trees to get away.
