I think you've made an assumption to far based on scanty information.
HQ (and HW before it) battles are as bloody and viscious as /you/ want them to be. It is up to the GM and players to provide that kind of color. You evaluate the player's success vs the defender's and decide what the most appropriate outcome is.
If your player is cautious, and wins by betting his minimum skill (advantage points) each round, he probably just exhausted the opponent, or bored him, into giving it.
If your player goes whole hog and declares "I take his friggin head off!" and backs up the claim with a suitable expenditure of advantage -- and wins -- then -- by golly -- he takes that opponents friggin head of in a glorious shower of blood and gore, if you so desire.
Some of the best combats I have ever witnessed occured in Hero Wars. I can only presume that they will be equally impressive in HQ.
As far as basket weaving vs spear...it doesn't at first sound pratical, but if my hero has basket weaving at high levels, say 15W3, what's to say that he can't
1) have previously woven an impressive set of reed armor, thus giving him a method to use his skill as defense against the spear
2) entrap the spear-user by quickly weaving a "chinese thumb trap" around his hands and the haft of the spear, assuming he happened to have some spare reed or straw handy?
3) Use his precise knowledge of reeds to allow him to hold a single reed "just so" and ram it into the spear-user's eye and brain?
3) etc.
