Darrick's comments about Mage in that playtest thread (how that game portrays evil as "wimpy", and how the Paradox generated during the adventure would render it unplayable) made me think about something. According to the backstory of the Fiends, they were driven away from our reality a long time ago, transformed into demonic monsters in the realms beyond, and now sent back by their dark masters to corrupt the world, only to discover that their magic no longer affects mortals in the expected fashion because these don't believe in it any more... Is it just me, or does this sound an awful lot like a Nephandi chronicle? After all, every hell ever imagined exists somewhere in the Umbra where Paradox isn't so much of an issue: there's no reason why K'thana couldn't, as well.
