See "Automatic Failures and Successes" on page 136 of the 3.5 Player's Handbook.
This rule is not in Mutants & Masterminds, with the limited exception of the "Impossible Toughness Saves" rule: if your save bonus is high enough to succeed with a roll of 1, you succeed, if it's not high enough to succeed with a roll of 20, you fail (except for Toughness saves, where you just suffer the minimum damage, and only then for important characters).
