Now, in CoC BRP, that's basically done by GM fiat - some monster has an ability to pernamently remove CON, or the GM decides it happens because of some unscripted thing - "You're caught in the machinery and can't pull your arm out as it's slowly crushed, take 3 points of CON damage and why not make a san check?"
I figure the same thing can happen in d20 - admitedly, it's not too common, but there are D&D undead monsters that pernamently damage Con, so there's no reason there can't be d20 CoC who do the same thing.
So, when I was doing my 'experienced adventurer fight' comparison, I mocked up similar Con damage on both, only to see that the con damage in d20 seemed to be more troublesome than the same con damage in BRP.
