Oh, there were certainly deaths -- even wipes on occasion. I said the traps were avoidable, not that they were always avoided.

Situations like that are deaths with only the tactics stated, and wipes if the party moves together as a whole unit (in what my gaming group calls "Charlie Foxtrot Formation".) More usually, part of the group moved forward and part of the group -- including at least one raise dead-capable cleric -- remains back in a cleared and safe part of the dungeon.

I think that a certain assumed body-count is characteristic of the era. After all, assumed party sizes were 8-12 characters, plus an average of 4 henchpeople per character varying by charisma, plus hirelings and men-at-arms. You'd expect to lose some for a treasure haul of that magnitude.
