Yep, and it's the biggest balance thing to watch out for. I do like the smaller limits <I>Questers</I> encourages.

There's no such thing necessarily. If you're using a Quality like, say, Warrior or Fencer then it's assumed you have all of the tools you need to use that Quality. So, by default, you have a sword or whatever your weapon of choice is and use it while rolling that Quality. Damage in all versions of PDQ is either Failure or Damage ranks inflicted on the other character based on the difference in the two rolls - so a 15 on the attack vs. a 10 results in 5 Ranks of damage, which lowers Qualities until they all hit Poor -2 and the other character Zeroes Out.
If you wanted to play up the importance of a weapon - perhaps a magical heirloom - then you could buy it as a Quality. Frostwind +2*, for instance, could represent a magical sword of ice with various powers that can be called on. You'd add that to, say, your Warrior Quality when attacking with it.
